LESLIE JEWELL, RN, MSN, ADMINISTRATOR
Leslie is the owner and Administrator of Community Choice Home Care, Inc. She established the agency in 1996 and has been a nurse since 1991.
JOSH SMITH, ASSISTANT ADMINISTRATOR, INFORMATION TECHNOLOGY MANAGER
Josh has been with Community Choice Home Care, Inc. since 2000 as Payroll and Billing Coordinator. Josh is Microsoft™ Certified since 2011.
MELISSA SMITH, RN, BSN, MHA CLINICAL DIRECTOR
Missy Smith has been an employee with Community Choice Home Care, Inc. since 2001. She began working as a home health aide while attending nursing school and after obtaining her Associate degree she began working full time as a registered nurse. Missy received her BSN in 2009 and her Master's in Health Administration in 2012.
